Benefits Carrier Invoice Reconciliation

A carrier bill is not proof that enrollment is correct. Reconciliation compares the invoice with approved eligibility, elections, effective dates, employer contributions, and payroll deductions before payment.

Four-way reconciliation

Record What it proves

Enrollment platform or carrier Coverage tier, option, members, and carrier-effective dates
Payroll Employee deductions and employer contributions actually processed
Carrier invoice Members and premiums billed for the coverage period

Common discrepancies

  • Terminated employees or dependents still billed.
  • New enrollments missing from the invoice.
  • Wrong coverage tier, plan option, age band, or tobacco status.
  • Retroactive adds and terms without matching credits or charges.
  • COBRA participants billed to the active population.
  • Payroll deductions beginning or ending on the wrong pay period.
  • Rate changes or employer contributions configured incorrectly.

Monthly close

  1. Freeze source reports for the same coverage period.
  2. Match members and premiums at the lowest practical level.
  3. Research variances with documented ownership and aging.
  4. Submit eligibility corrections and track expected credits.
  5. Approve payment under a defined materiality and escalation policy.
  6. Reconcile payroll deductions and employer expense to the final liability.
  7. Retain the reports, variance log, approvals, corrections, and credit follow-up.
Control principle: Paying an invoice on time and paying the correct invoice are separate objectives. The process must protect coverage continuity while unresolved discrepancies remain visible.
